A New Onset of Ulcerative Colitis post-2nd-breakthrough-COVID-19

A 21-year-old non-smoking Caucasian male with a history of acute pancreatitis but no other medical issues or family history had two doses of the BNT162b2 mRNA COVID-19 vaccine.  Four months after the second dose he had his first episode of COVID-19. Although not hypoxic, he felt pretty unwell for a week, with a severe cough, fever, generalized body ache, headache, and loss of taste. He received the third dose of the vaccine two months after recovering from COVID. Nine months after the third dose, he had the second episode of COVID-19, during which he was mildly unwell for three days, recovered, and did not require any anti-viral medication or antibiotics. One week post the second episode of COVID-19, he developed diarrhea and abdominal pain. It then progressed to bloody diarrhea. Polyarteritis nodosa (PAN) is a rare, but serious autoimmune disease that affects the medium-sized arteries, can block the flow of blood and nutrients and affect many different organs in the body, including the skin, joints, nerves, kidneys, and gastrointestinal tract.  It is a complex condition with multiple genetic and environmental factors likely playing a role. Genetic factors that have been associated with PAN include variations in MEFV, TNFAIP3, CECR1, DADA2, and ITGB2 genes.  A healthy 59-year-old woman presented to the emergency department with a two-day history of progressively worsening abdominal pain, melena, and postprandial nausea. Pancreatitis after COVID-19 vaccines

A 24-year-old South-Asian female received  er first dose of the Pfizer vaccine  at 30 weeks of gestation. One week later she presented with severe epigastric pain radiating to the back and worsening on lying supine, associated with nausea and vomiting. She was diagnosed with acute pancreatitis with a serum lipase level of 4376 U/L and an ultrasound showing features of pancreatitis. The patient had a spontaneous vaginal delivery, and the baby was shifted to the neonatal intensive care unit in a stable condition. A computed tomography scan postpartum (day 2) demonstrated acute interstitial edematous pancreatitis. The patient was managed conservatively in the intensive care unit and discharged home in a stable condition. Systemic adverse reactions after the 5th dose of COVID-19 vaccine

As fewer and fewer people opt for additional boosters of COVID-19 vaccine, are those who never had vaccine adverse events experiencing new side effects? VAERS reports many uneventful vaccinations even with extra doses given too soon after previous ones. Yet there are cases such as  abdominal pain , or Addisonian crisis  after the 5th dose. One case of systemic adverse reactions (14%, out of 7 respondents) was self-reported after the 5th booster in a survey-based study . The study also showed higher incidence of cutaneous reactions after 3rd vs 2nd dose of the same vaccine.  89 out of  234  vaccinated individuals  (38%)   who filled out  a web-based survey in Turkey  reported  at least one cutaneous adverse event related to the vaccination.  The responders cumulatively received 771 doses.  The mean number of vaccine dosages was 3.29 ± 0.76 (minimum:1, maximum:5). Prolonged Diarrhea after COVID-19 vaccination

A 46-year-old woman with no notable medical history developed repeated vomiting approximately four hours after receiving the first BNT162b2 mRNA COVID-19 vaccine. Seven days after the first vaccination, she further developed 3–5 loose-to-liquid stools per day, which corresponded to type 6 on the Bristol Stool Form (BSF).  Two weeks after the first vaccine dose, intermittent paraumbilical pain developed with associated loss of appetite. The patient decided to receive the second dose 21 days after the first dose, which furthered worsened her condition.  Vomiting and diarrhea progressed, with 7–10 watery stools per day (BSF type 7). Blood tests performed 4 weeks after the second dose revealed normal levels of total and differential white blood cell (WBC) counts and serum C-reactive protein (CRP), slightly elevated D-dimer serum level (1.1 μg/mL), and slightly elevated serum C1 esterase inactivator activity.
