A 13-year-old boy presented to the hospital with a complaint of painless diminution of vision in the left eye for 2 weeks. It started 15 days after taking the first dose of Corbevax COVID-19 vaccine. Corbevax, also known as BACOV2A, is a  recombinant protein subunit vaccine  approved for emergency use in India  o n 28 December 2021. Fundus fluorescein angiography of the left eye showed a delay in venous filling in the early phase of angiogram and blocked fluorescence corresponding to the areas of hemorrhages revealed by fundus examination.  The patient was administered three doses of pulse corticosteroids followed by a tapering dose of oral corticosteroids. We discuss 29 cases with a median age 82.  A   78-year-old man went to the Emergency Department at King Fahad Medical City in Riyadh, Saudi Arabia, with numerous bullae throughout his extremities one day after receiving the second dose of Pfizer Covid-19 vaccine. The bullae were preceded by intense pruritus and urticated plaques. A  skin biopsy  was performed which revealed IgG (+1), IgM (+1), and C3 (+1) staining of the  basement membrane . The patient was treated with topical and systemic corticosteroids, fusidic acid  cream, and emollients after a confirmed diagnosis of bullous pemphigoid (BP) was obtained. In the period 27 December 2020 to 15 February 2021, about 29 400 of Norway's nursing home patients (aged  61–103)  were vaccinated with the mRNA vaccine BNT162b2. During the same period, the Norwegian Medicines Agency received 100 reports of suspected fatal adverse reactions to the vaccine. An expert group has examined the 100 reported deaths and concluded that 10 (10 %) are most likely related to the vaccine while there could be a possible link for other 26 individuals (26 %).  A  recent review  of other 38  cases linking the COVID-19 vaccines and the death found the following causes of death: vaccine-induced immune thrombotic thrombocytopenia (VITT) (32), myocarditis (3), acute disseminated encephalomyelitis (ADEM) (1), myocardial infarction (1), and rhabdomyolysis (1) . SUMMARY   We report a case from an ongoing COVID-19 vaccines long-term safety study . We describe a fatal SARS-CoV-2 breakthrough infection occurring after a serious adverse reaction to ChAdOx1 nCov-19 vaccine. Two years into the outbreak, COVID-19 treatment and preventive care are still not sufficiently personalized. Radiographic and laboratory findings commonly used to monitor progression to severe disease fail to select appropriate treatment or subsequent immunization regimen while predicting response to therapy remains a clinical challenge. Retrospectively analyzing post-immunization adverse events helps in understanding the relationship between host susceptibility and vaccine efficacy, which may ultimately lead to improved interventions.
